Compensation: ~$90-100k + equity and bonus tied to key deliverables

The Role

The Patient Care Coordinator plays a critical role in the patient's surgical journey. This individual will educate prospective patients, guide them through the enrollment process, coordinate with the clinical team, and ensure a positive patient experience from first touch through the hand off to the clinical team. The ideal candidate is tech savvy, personable, organized, super engaging, and deeply committed to using technology to improve patient access to quality care.

Key Responsibilities

Engagement and Care Coordination

  • Maintain a caseload of patients during onboarding, eligibility, and medical record collection process
  • Manage inbound and outbound calls, proactively engaging eligible patients to enroll in their World Class Health surgical benefit
  • Educate patients on program eligibility, benefits, and enrollment steps in a clear and proactive manner
  • Assist patients in completing enrollment forms, collecting required documentation, and navigating digital systems or portals as needed
  • Work closely with nurses, operations, and administrative staff to ensure seamless patient handoffs.
  • Share insights from patient interactions to help inform operational improvements.
  • Coordinate appointments, scheduling, and communication between patients, nurses, and providers.
  • Provide high-touch support with empathy, ensuring patients feel informed and cared for throughout their journey.
  • Track and resolve patient issues, escalating when needed to clinical or operational leadership.
  • Leverage the latest technology and platforms to find gaps in care and work in collaboration with the ops team to implement new systems or improve current ones!
Qualifications
  • 2+ years of experience in start up healthcare sales, patient services, or care coordination.
  • Mandatory prior startup experience — proven ability to thrive in fast-paced, ambiguous environments with limited existing structure.
  • Outgoing personality, sales mindset, ability to connect with patients seamlessly
  • Extensive experience using Gsuite, Salesforce and other relevant platforms
  • Tech-forward and AI-energized — you’re genuinely excited by emerging technology and driven to be at the leading edge of what’s next.
  • Must be able to be in office in SF 6 days/mo
Preferred
  • LVN or RN with clinical experience in remote patient care
  • Prior experience working for a Center of Excellence program
  • Understanding of the surgical process for ortho, cardiac, bariatric, spine, and cancer procedures
  • Bilingual Spanish or Hindi
